NATIONWIDE SMALL PET TRANSPORTS 59154 Trafton Lane - JOHN DAY, OR 97845 Transport Checklist We would like to thank you for transporting with Professional Pet Transports, Inc. Please take a moment to go over this checklist in order to make your pet(s)'s transport go smoothly. 1) ___ An Interstate Health Certificate is a form distributed by the state of origin Department of Agriculture and completed, dated and signed by a licensed Veterinarian. A computer-generated form is not considered an Interstate Health Certificate. This is a requirement of the USDA. Also be sure that if you are sending a dog, that he/she has had a Bordetella (kennel cough) shot as well. 2) ___ If you are sending food with your pet(s), please have only enough food for the transport in one or two zip-lock bags for each pet, depending on the size of your pet(s). Please also include specific, written feeding instructions for your pet(s). 3) ___ Please only have one toy or article of clothing per pet. 4) ___ Please do not use a quick-release collar as these have a tendency to break. The collar you use should fit your pet well but not be too tight or too loose. 5) ___ Please do not send any bowls or extra leashes or collars with your pet(s) as we do not have room on our buses for them. 6) ___ Please have a pillow-type bed ready to go with your pet or arrange to purchase one through our company. If your pet is under six months of age or an older pet with possible accident problems, please be sure the bed is completely waterproof. If you would like to order a bed from our company, please call us toll-free at 1-866-ARE-PETS (273-7387) at least 24 hrs prior to our buses leaving their points of origin to ensure the bed is on our bus when the transport begins. 7) ___ If your pet is on medication, please send only the necessary medication needed for the transport. 8) We would like to remind you of our one-of-a-kind Pet Tracker. This service is provided for an extra $25 and will enable you to watch your pet's journey. We try to update Tracker at least four to six times a day, depending on cell phone service. Tracker will show you where the bus carrying your pet is, what the weather and road conditions are like and comments from our drivers about your pet(s). To access Pet Tracker, find your Order Number (in red) at the top of your invoice. Input this number on our home page (http://www.pro-pet-transports.com) where it asks for Invoice Number at the top of our menu bar and press the arrow button on the right of the input box. 9) We would also like to remind you of our Journey Pictures option. If you would like for our drivers to take pictures of your pet on the journey, call and let us know prior to beginning of scheduled transport week. 10) If you have ordered a Deluxe Package, your pet's Toy and Pet Bed will already be on board before buses leave. Pet Tracker will also be turned on for you prior to pickup. 11) Please write a list of any habits, personality traits, medication details and anything else you feel our drivers should know about your pets during the transport. This will help to ensure your pets receive what they need to make the trip as comfortable and enjoyable for them as possible. |
